Custom light fixture design is often a part of a complete design environment, Here are some examples

The strategically placed internal supports cast a diagonal shadow to create a diagonal pattern across the ceiling,

Sometimes the driving force for a custom light is functional as in the glass enclosed fluorescent fixture above.The adjacent high window (one of many) has an arched top above the top of the wall. Low angle sunlight would have disturbed neighboring work areas, but the glass panel blocks direct sunshine while giving a diffuse glow to the setting sunlight. It seemed only appropriate to make it a light fixture also.
